Death Certificates

The form includes applicable fees, identification requirements, and mailing the address. The cost for the first certificate is $8.00 and $4.00 for each additional copy. Please make checks payable to the Indiana Department of Health.

Any physician, or covering physician, with treatment responsibilities or who has written any prescription (even if written without a visit) for the patient within the previous 12 months is considered an attending physician and is required to sign a death certificate within 24 hours.

The price for a death certificate varies from county to county. The price can range from $5.00 to $25.00. All states use different prices.

The Indiana Department of Health issues certified copies of Indiana death certificates, as well as birth, marriage, and divorce certificates. You can only get a vital record in Indiana for an event that occurred in Indiana. In other words, you can only get an Indiana death certificate for a person who died in Indiana.
